GENERAL SUMMARY OF SCOPE – The Jacksonville Port Authority (JAXPORT) is soliciting proposals from firms or entities interested in and capable of serving as the WELLNESS PROGRAM ADMINISTRATOR for JAXPORT’s Wellness Program.  This is an evaluated contract and may be awarded to one or two Proposer(s) receiving the highest number of points by proving their ability to meet both entities needs and requirements as outlined on the Evaluation Criteria. The successful firm(s) and/or administrator(s) must demonstrate the capacity to provide wellness program services and initiatives that promotes a culture of healthy lifestyle choices.  Therefore, the purpose of this Request for Proposal (RFP) is to obtain a pricing commitment in regards to your ability to have the necessary organization, experience, and staff to provide these services exactly as specified.

QUESTIONS DEADLINE – The deadline for submitting questions by e-mail to Jerrie Gunder at jerrie.gunder@jaxport.com or submitted through E-Builder is MONDAY, OCTOBER 19, 2020 at 9:00 AM (EST).

PROPOSALS DUE – before 2:00 PM (EST) on TUESDAY, OCTOBER 27, 2020.  Proposals and all required supplemental material must be electronically submitted in PDF format only through E-Builder.

Until further notice, JAXPORT is not accepting any Proposal packages submitted by Mail or Hand-Deliveries due to the current COVID-19 situation.  Any Proposals/Addenda submitted by Email, Fax, Mail or Hand-Deliveries, will not be accepted or considered.
